INFORMATION ABOUT AND REQUEST OF CONSENT
TO THE TREATMENT OF PERSONAL DATA

under Article 13 of the Legislative Decree n° 196 of 2003.30.06

Please read carefully the following Information before providing us with your personal data, specifying that “Personal Data”, as provided for in the Legislative Decree 196/2003 and as explained in this information, are:

• all data that we request you through this form and any further data that we will need to obtain in the course of this agreement or during the fulfilment of services in your favour;
• all data that you will spontaneously provide us, specifying that you are not forced to provide them;
• no sensitive or legal data will be registered nor treated, as they are not necessary for this agreement.

• 1. All data are handled in order to allow an efficient management of the professional and commercial activities that we provide you with, including the administrative and account management; especially, some Data will be regularly registered in accounting archives or any other kind of database compulsory by law.
• 2. All data are handled both manually, whereas the respective paper files will be correctly held and kept with care during the whole period necessary to the execution of the treatment, and by means of our automated information system, whereas they will be registered on protected information supports.
• 3. The collected and processed data can be:

• made available to the collaborators of the Data controller, both Responsible or Appointed persons;
• forwarded to physical or legal third parties, only if these are involved in and functional to the management of the activities requested to us or transferred to another Data controller for outwards treatments, in any aspect that may concern them and according to the instructions provided by law
• the Data will not be object of any diffusion and they will be destroyed once we will not need to keep them anymore;

• 4. We would like to inform you that, as the Concerned Person, you have all the rights provided by art.7 of the Legislative Decree n. 196/2003, that's to say:

• A – the right to obtain from the Data controller and with no delay:
  • confirmation of the existence or non existence of personal information which regard you, their update, correction, integration, cancellation, transformation into anonymous form or the block of data treated in violation of law;
  • the attestation that the operations mentioned above have been brought to knowledge to those to whom the Data have been communicated, except in cases in which such an action is impossible or requires an un-proportioned employment of means compared to the right being safeguarded.
• B – the right to object totally or partially, for legitimate reasons, to the treatment of personal information which regard you, even if it is pertinent to the aim it is being required for., being aware that, in this case, we will not be able anymore to provide you with our professional services.
  All rights referred to in art. 7 of Legislative Decree  n. 196/2003 are exercised under request not formally made to the Responsible of the Data treatment or to the Data Security Manager, even through an Appointed person, that must be noticed with no delay.

• 5. The holder of the rights to manage such data is STUDIO3TV di Trovellesi Cesana Luca & C Sas, with its offices at Porto S. Elpidio (AP) in Via Fontanella n° 44, represented by its legal pro tempore responsible and Data Security Manager Mr Trovellesi Cesana Luca, with address for service in the offices of Studio3 TV, according to the regulations provided for in the Legislative Decree 196/2003, whereas it is also available the updated list of the external subjects to whom the Data could be communicated.
